We do three things. They are the same thing, in three places.
Most operations don't break in one spot. They lose value in the gaps between the parts that work. Demand arrives faster than anyone can answer it. What gets answered depends on someone remembering. And the tools that were supposed to hold it together were built for a business shaped like everyone else's, not yours. Three gaps, one cause. Nothing owns the space between the moving parts. That space is the whole of what we work on.
